Live Well Brantford-Brant making a meaningful difference to our most vulnerable citizens

BRANTFORD, ON – Consistent with the Brantford-Brant Housing Stability Plan, the City of Brantford is committed to continually improving the conditions that enable all members of our community to enjoy the highest level of health and well-being possible.

The Live Well Brantford-Brant (LWBB) Program provides supportive housing with intense case management for individuals aged 16 and older with high needs related to physical or mental health, developmental disabilities or substance use. Clients involved with this program have experienced repeated or chronic homelessness and work with staff to transition into independent living in the long-term. The program is 100 per cent provincially funded through the Homelessness Prevention Program that focuses on the goal of reducing homelessness by targeting specific populations who have typically endured the most difficulty obtaining and retaining housing.

Since 2020, the LWBB Program has housed 102 individuals, including 53 youth and 49 adults.  Several individuals have successfully exited the program.

“As evidenced, individuals with complex needs who receive this type of support have better outcomes,” says Mary Musson, Director of Housing and Homelessness, City of Brantford. “Staff that administer the Live Well Brantford-Brant Program have made a significant difference in the lives of participants resulting in a positive impact in our community and households that need it the most.”

In 2022, the LWBB Adult Program provided housing for nine new tenants, supported five tenants in transferring to more independent living, and had two clients successfully complete the LWBB Adult Program. Over 270 youth have been supported through the LWBB Youth Program by St. Leonard’s Community Services in 2022.

The LWBB Program will expand in 2023 to provide increased services and support to more households in need. This means that more individuals and families will be able to access additional wraparound supports while continuing to live independently.
